sudo -H gem cleanup does not remove some old gem versions.

e.g., after running the above, both 0.4.2 & 0.4.6 of io-console remain, but the following shows there are no dependencies on either, so I don't know why 0.4.2 wasn't removed.

gem dependency io-console --reverse-dependencies
Gem io-console-0.4.2

Gem io-console-0.4.6

Can anyone explain this behavior / help fix it?

    Hi Ross,

io-console is a default gem (gem which comes bundled with ruby). It is expected behavior of gem cleanup to not remove default gems. Please checkout #1530.

Uninstalling default gems is dangerous and not recommended. Checkout this stackover question if you know what you are doing.
